What proof means here: If the Estate previously was open but has since been closed, you may provide a closed estate affidavit or a closing statement issued by the court. You also will need to provide a copy of the Vested Employee’s Death Certificate and a copy of the Letters Testamentary or other court-issued documentation to show us that you were appointed as personal representative or executor of the estate. If an estate was never opened, you may provide a copy of the Vested Employee’s Death Certificate and a small estate affidavit attesting to who is entitled to receive the Vested Employee’s assets.
